You are eligible for a new passport:
Required when applying for your first passport.
your previous passport was issued at under 16 years of age
the previous passport issued more than 15 years ago
It says I need to appear in person, isn’t that what you are doing for me?
It is necessary for an acceptance agent to verify your identity (DO NOT sign the application until asked to do so). The agent will seal the appropriate documents into an envelope. It is this envelope that you will send to me for expediting.
Why do our children have to appear or if we are signing for them?
The agent has to look at the child and compare him or her with the photo. It is a very important part to help prevent child abduction.
Is there any reason my passport application could be denied?
Passport applications may be denied for various reasons. The most common is having missing necessary documents such as proper proof of citizenship, proper photographs, or correct fees. Also, passports may be denied for certain legal issues. Be advised the US Department of State has the right to request additional documents at its discretion.
What is a passport card?
A passport card is a wallet-sized travel document. It can be used for land or sea travel to Canada or Mexico as well as sea travel to the Caribbean and Bermuda. It will not get you on an international flight. It is also good as a form of ID in the United States.
What if my name has changed since I received my current passport?
If you need to change the name on your passport and your passport is less than 1 year old, use Form DS-5504. If the current passport is more than a year old, you need to do a renewal form DS-82 (passport renewal.) If your ID has changed or if your tickets are in your new name, you should get an updated passport prior to your trip. You will need to submit the original court document showing the name change along with your current passport.

Do I need a U.S. passport to travel?
You will need a passport to travel every time you leave or enter the USA. The only exception is when you are traveling by land or sea to Canada, Mexico, the Caribbean, and Bermuda – for this you can use a passport card.
